DCBS Media ReleaseNovember 16, 1999 State revokes license of Eugene insurance agentSALEM – The Department of Consumer and Business Services (DCBS) has revoked the insurance agent's license of a Eugene man, Trevis W. Luzier, for violations of the Oregon Insurance Code. Luzier also was fined $5,500. An investigation by the Insurance Division of DCBS found that Luzier had altered information on four applications for life insurance policies without the knowledge or authorization of the applicants. Luzier also failed to notify the Insurance Division of changes in his residence address and telephone number, and did not respond to an inquiry by the division.
