Licensing Application Procedures
Individual Producers
Transferring from Another State
I. Applying within 90 days of establishing residence:
NOTE: This applies only to producers who move to Oregon and apply for a resident license within
90 days of establishing legal residence in this state.
To transfer a license from another state: Please indicate the date you legally established residency
in Oregon.
- Submit an Individual
Insurance License Application (Form 3000).
- Submit fingerprint information. Fingerprints may be digitally taken at the test center, or
submit
a fingerprint card.
- Submit the Criminal
Records Request form (Form 4862).
- Pay all required fees.
NOTE: No exam is required if the applicant is applying for the same lines
he or she was licensed for in the previous home state. Applicants for variable life must be licensed
to a securities dealer in Oregon.
II. Applying more than 90 days after establishing residence:
NOTE: This applies to producers who apply for a resident license more
than 90 days after they established legal residence in Oregon.
To transfer a license from another state:
- Submit an Individual
Insurance License Application (Form 3000).
- Have fingerprints digitally taken at the testing center, or submit
a fingerprint card.
- Submit the Criminal
Records Request form (Form 4862).
- Pay all required fees.
NOTE: Pre-licensing training is required for applicants who have less
than three years of producer experience in the lines applied for. Appropriate exams also are
required.
